Sleep specialists across Maryland, D.C. and Virginia, and why the drive is the problem

CareFirst is the Blue Cross licensee for Maryland, the District of Columbia and Northern Virginia. Across those three jurisdictions the American Board of Sleep Medicine lists nine behavioral sleep medicine specialists in total — but only eight sit inside CareFirst's own territory, and all eight are in the Baltimore–Washington corridor. The ninth, Virginia's only listing, is in Charlottesville, well outside it. CareFirst members are either almost on top of a specialist or nowhere near one.

  • Virginia has one listed specialist, and it isn't in Northern Virginia: The state's single entry is in Charlottesville. Northern Virginia — CareFirst's Virginia territory, and home to more than two million people — has none of its own. Its members' nearest options are across the river in the District, Bethesda or Silver Spring, which turns a routine appointment into a Potomac crossing.
  • Corridor traffic makes a short trip a long afternoon: Maryland's seven listings cluster in Baltimore, Bethesda, Silver Spring, Potomac and Woodbine, with one in the District. On paper that is dense coverage. In practice the Washington–Baltimore corridor is one of the most congested commutes in the country, and a twelve-mile crosstown appointment routinely costs an afternoon of leave rather than an hour.
  • The District has one listing, for a city that doubles by day: Washington's single entry serves a resident population around 700,000 that swells past a million on a weekday as the commuter workforce arrives. It is also the closest option for Northern Virginia members, which means one certified specialist is the practical answer for a good part of two jurisdictions at once.
  • The gap is uneven: A member in Bethesda has several options within a few miles. A member in Western Maryland or on the Eastern Shore has none within a half-day's drive — same insurance card, different access entirely.
